Having a baby in Japan means coordinating several institutions. This guide maps which office handles pregnancy notification, birth notification, childbirth lump-sum, maternity allowance, childcare leave benefit, Child Allowance, and Tokyo support.

In Japan, baby paperwork is split across several institutions. Your ward or city office handles local steps like pregnancy notification, Mother and Child Health Handbook, birth notification, Child Allowance, and many daycare or local child-support procedures. Your employer handles leave dates, HR forms, wage certificates, and often the childcare leave benefit paperwork flow. Your health insurer handles childbirth lump-sum payment and, for eligible insured employees, maternity allowance. Hello Work handles childcare leave benefits under Employment Insurance. Your hospital or clinic handles medical care, birth certificate paperwork, delivery billing, and sometimes direct-payment paperwork with your insurer.

Ward or city office: pregnancy, birth, Child Allowance, and local support

Your ward or city office is the first place to think of for local family paperwork.

After pregnancy is confirmed, the pregnancy notification (妊娠届) is a municipal step. The Children and Families Agency handbook guide explains that a pregnant person should notify the municipality promptly, and the municipality issues the Mother and Child Health Handbook (母子健康手帳) after the notification. The same guide explains that the handbook process connects pregnant people to municipal maternal health services and may include prenatal health checkup tickets, subsidy vouchers, or service information. It also notes that if you move, vouchers from the previous municipality may become unusable and you should contact the new municipality. (Children and Families Agency)

After birth, the birth notification (出生届) is also a municipal procedure. The Ministry of Justice lists submission within 14 days for a birth in Japan, with submission to the city/ward/town/village office of the child’s birthplace, registered domicile, or notifier’s location. The MoJ page also lists one birth certificate as an attachment. (Ministry of Justice Japan)

Child Allowance (児童手当) is also usually a municipality step. The Children and Families Agency says that after a child is born or after moving to another municipality, the recognition claim is submitted to the current municipality; public servants apply through their workplace. (Children and Families Agency) For a birth, the Children and Families Agency says the application is needed at the current municipality within 15 days from the day after birth, and late application can generally mean missing monthly benefits. Health insurer: childbirth lump-sum and maternity allowance

Your health insurer is the key owner for health-insurance benefits.

The childbirth lump-sum payment (出産育児一時金) is generally ¥500,000 per child when official requirements are met. MHLW explains that it is paid by the health insurer, and that direct payment may be used so the insurer pays the birth facility directly, depending on the facility’s handling. (Ministry of Health, Labour and Welfare)

MHLW also lists the application deadline for childbirth lump-sum as within two years from the day after birth. (Ministry of Health, Labour and Welfare) In practice, ask both your hospital and insurer whether the direct-payment system (直接支払制度) is available, what you must sign, and what happens if the birth cost is higher or lower than the lump-sum amount.

Maternity allowance (出産手当金) is different. It is not a ward office benefit. Kyokai Kenpo explains it as a health-insurance benefit for eligible insured persons who are off work around childbirth and whose salary is not paid. Kyokai Kenpo lists the covered period as 42 days before birth or due date, 98 days before for multiple births, through 56 days after birth. (Kyōkaikenpo)

Kyokai Kenpo explains the maternity allowance daily amount as the average standard monthly remuneration over the prior 12 months divided by 30, multiplied by two-thirds, with special handling when fewer than 12 months are available. Employer / HR: leave dates, certificates, payroll, and often benefit paperwork

Your employer or HR team owns the workplace process. This includes leave requests, company-specific forms, HR/payroll certificates, wage records, and coordination with social insurance and Employment Insurance procedures.

MHLW’s maternity/childcare leave tool uses expected birth date or desired childcare leave start date to show legal application timing and leave period under Japanese labor-related rules. For employee social-insurance premium exemption during maternity leave, the Japan Pension Service says the employer files after a request from the insured person. Hello Work: childcare leave benefit under Employment Insurance

Hello Work is the important owner for childcare leave benefits under Employment Insurance.

MHLW’s childcare leave benefits page lists childcare leave benefit-related categories and tools, including a simulation tool and information about changed extension procedures from April 2025. (Ministry of Health, Labour and Welfare) MHLW also states that the application destination is the Hello Work office with jurisdiction over the employer’s business office location. The detailed MHLW PDF says the submitter is normally the employer of the insured person, while the insured person may submit directly if desired. (Ministry of Health, Labour and Welfare) It also lists supporting materials such as wage ledgers, worker rosters, attendance records, childcare leave request/notice, and proof such as Mother and Child Health Handbook pages or residence records, depending on the application context. MHLW’s PDF explains childcare leave benefit as wage daily amount × payment days × 67%, with 50% after day 181. (Ministry of Health, Labour and Welfare) Treat this as a basis for understanding, not a final amount. Your employment status, wage basis, leave dates, caps, and official review can affect the outcome.

MHLW notes that the childcare leave benefit call center cannot answer exact payment dates. (Ministry of Health, Labour and Welfare) For your family calendar, ask HR what the expected filing cycle is and what confirmation HR receives from Hello Work.

Hospital or clinic: birth certificate, delivery costs, and direct payment

Your hospital or clinic is not usually the owner of ward benefits or leave benefits, but it matters for several key steps.

The Ministry of Justice birth notification page lists one birth certificate as an attachment for the birth notification. (Ministry of Justice Japan) In practice, the birth certificate section is usually handled through the hospital or clinic, so ask the facility how and when you receive the completed document.

For childbirth lump-sum direct payment, MHLW explains that under direct payment, the birth facility applies to the insurer and the insurer pays the facility directly. (Ministry of Health, Labour and Welfare) Ask your hospital billing desk whether it supports direct payment, what form you need to sign, and how settlement works if the final cost differs from the lump-sum amount.

Tokyo Metropolitan Government: Tokyo-wide programs and tools

If you live in Tokyo, some support is Tokyo-wide, while other details are still handled by your ward or municipality.

Tokyo’s 018 Support page says FY2026 continues, with ¥5,000 per month, up to ¥60,000 annually, for children meeting listed Tokyo residence and age conditions. (Tokyo Welfare Bureau) Confirm the official application route and documents through Tokyo’s official 018 Support page or portal.

Tokyo also says that from Sept. 1, 2025, it supports municipalities implementing first-child childcare fee free for households using certified childcare facilities, regardless of age or income. (Tokyo Welfare Bureau) Because this is implemented through municipalities, confirm with your ward or city.

For daycare search and hoikatsu, Tokyo’s Hoikatsu One-stop Project supports daycare facility search, tour booking, and access to municipal application/procedure pages in participating municipalities.
