Your ward or city office is the first place to think of for local family paperwork.

After pregnancy is confirmed, the pregnancy notification (妊娠届) is a municipal step. The Ministry of Justice lists submission within 14 days for a birth in Japan, with submission to the city/ward/town/village office of the child’s birthplace, registered domicile, or notifier’s location. Child Allowance (児童手当) is also usually a municipality step. The Children and Families Agency says that after a child is born or after moving to another municipality, the recognition claim is submitted to the current municipality; public servants apply through their workplace. Health insurer: childbirth lump-sum and maternity allowance
Your health insurer is the key owner for health-insurance benefits.
The childbirth lump-sum payment (出産育児一時金) is generally ¥500,000 per child when official requirements are met. MHLW explains that it is paid by the health insurer, and that direct payment may be used so the insurer pays the birth facility directly, depending on the facility’s handling. MHLW also lists the application deadline for childbirth lump-sum as within two years from the day after birth. (Ministry of Health, Labour and Welfare) In practice, ask both your hospital and insurer whether the direct-payment system (直接支払制度) is available, what you must sign, and what happens if the birth cost is higher or lower than the lump-sum amount.
Maternity allowance (出産手当金) is different. It is not a ward office benefit. Kyokai Kenpo explains it as a health-insurance benefit for eligible insured persons who are off work around childbirth and whose salary is not paid. Kyokai Kenpo lists the covered period as 42 days before birth or due date, 98 days before for multiple births, through 56 days after birth. (Kyōkaikenpo)
Kyokai Kenpo explains the maternity allowance daily amount as the average standard monthly remuneration over the prior 12 months divided by 30, multiplied by two-thirds, with special handling when fewer than 12 months are available. Hospital or clinic: birth certificate, delivery costs, and direct payment
Your hospital or clinic is not usually the owner of ward benefits or leave benefits, but it matters for several key steps.
The Ministry of Justice birth notification page lists one birth certificate as an attachment for the birth notification. For childbirth lump-sum direct payment, MHLW explains that under direct payment, the birth facility applies to the insurer and the insurer pays the facility directly. Tokyo Metropolitan Government: Tokyo-wide programs and tools
If you live in Tokyo, some support is Tokyo-wide, while other details are still handled by your ward or municipality.
Tokyo’s 018 Support page says FY2026 continues, with ¥5,000 per month, up to ¥60,000 annually, for children meeting listed Tokyo residence and age conditions. (Tokyo Welfare Bureau) Confirm the official application route and documents through Tokyo’s official 018 Support page or portal.
Tokyo also says that from Sept. 1, 2025, it supports municipalities implementing first-child childcare fee free for households using certified childcare facilities, regardless of age or income.
